Abstract
Pain abdomen in young unmarried female which is acute in onset draws attention to multiple differential diagnosis, especially in reproductive age group. Ruling out uterine, adnexal, conceptional pathologies clinically, biochemically and radiologically, the suspicion increases towards the intra-abdominal pathology.
A young lady reported with complaint of pain in lower abdomen for 03 days. The patient initially had pain around umbilicus which shifted to right iliac fossa, and hypogastric area. Her last menstrual period was 05 days prior to onset of pain. She denied history of previous abdominal surgeries.
Surgical removal of an Meckels Diverticulum is the current management of choice in a patient symptomatic meckels or for any of its complications. However, the controversy surrounding surgical removal of an incidentally detected meckels diverticulum still continues today.
Symptomatic Meckels diverticulum is rare in young females with age less than 15 years. Patients with acute abdomen with diagnostic dilemma on imaging with clinical condition not correlating with imaging radiology, high suspicion of meckels diverticular pathology should be kept in mind.

Key words: Meckels diverticulum, Heterotopic tissue, Diverticulectomy
