Introduction : L’hématome extradural est une urgence neurochirurgicale qui peut entraîner une hyperpression intracrânienne brutale et un effet de masse local, nécessitant une intervention urgente. Le diagnostic est radiologique et la prise en charge varie entre surveillance et chirurgie. Rapport de cas : nous rapportons le cas d’un patient hospitalisé au service de Neurochirurgie du CHU Mohammed VI de Tanger avec une évolution radiologique inhabituelle d’un hématome extradural. Il s’agit d’un patient âgé de 41 ans, admis pour des céphalées intenses, associée à des troubles de l’équilibre sans déficit moteur ou sensitif dans un contexte post-traumatique. Un scanner cérébral a été réalisé objectivant un hématome extra dural occipital gauche mesurant 29 mm, puis10 mm au bout de 24 h, avant de reprendre une taille de 19 mm après une semaine. Ce patient a été hospitalisé et surveillé au service avec bonne évolution clinique et radiologique. Le suivi à deux mois en consultation a trouvé un patient asymptomatique avec une TDM cérébrale de contrôle très satisfaisante. Conclusion : L’adoption de stratégies pluridisciplinaires concertées et réactives améliorent les pratiques et permet globalement de réduire la gravité des erreurs.


Abstract
Introduction: The extra epidural hematoma is a neurosurgical emergency that can lead to sudden intracranial hypertension and a local mass effect, requiring urgent intervention. Diagnosis is radiological and management varies between monitoring and surgery. Case report: we report the case of a patient hospitalized at the Neurosurgery Department of the Mohammed VI University Hospital of Tangier with an unusual radiological evolution of an occipital extradural hematoma. This is a 41-year-old patient, admitted for intense headaches, associated with balance disorders without motor or sensory deficit in a post-traumatic context. A cerebral scan was performed objectifying a left occipital extradural hematoma measuring 29 mm, then 10 mm after 24 hours, before resuming a size of 19 mm after one week. This patient was hospitalized and monitored in the department with good clinical and radiological evolution. The two-month follow-up in consultation found an asymptomatic patient with a very satisfactory control brain CT. Conclusion: The adoption of concerted and reactive multidisciplinary strategies improves practices and generally reduces the severity of errors.
